Job Description:

Job Duties:The Case Manager will be the liaison between the client and the community.Will be a link for clients by finding them volunteer work and community involvement.Will help clients work on resumes and look for work via Craigslist and on the internet.Will organize events for the clients in and around the community.Will organize outings, projects and field trips for clients.Will have access to guest speakers for groups for clients (NA, AA).Will hold monthly trainings for clients on :Universal Precautions,Client rightsEmergency proceduresGrievance procedureInfection ControlSTD's/HIV/AIDSClient wellnessWill coordinate outside Dr. appointments for clientsWill work closely with the MD in obtaining client medication prescriptions.Will keep in contact with the pharmacy to ensure timely delivery of client medications.Will work directly with the clinical Director for ideas, strategy and issues relating to clients.Will help clients with discharges, i.e. referring them to appropriate ancillary services working closely with the Clinical Director and specific therapist.Manage bus passes for clients.Manage client fundsComplete disability, food stamp and unemployment applicationsComplete orientation step downs from PHP to IOPTrack and manage client Rent receiptWill maintain a list of current client that need to see MDComplies with facility policies and procedures.Will participate in any and all trainings required for BTC staff according to regulatory guidelines.Participates in all staff meetings; responsible for obtaining information if not in attendance.Will work directly with the Clinical Director for ideas, strategy and issues relating to clients.Approaches clients/family members, significant others, and visitors with a respectful caring manner to convey a sense of concern and warmth.Maintains confidentiality of client information and records; and of facility proprietary, privileged, and/or confidential information.Communicates and problem solves via the chain of command.Delivers care in a non-judgmental and non-discriminatory manner, sensitive to client and staff diversity.Qualifications and Requirements:At a Minimum Associates Degree or equivalent preferred2-4 years of of work experience in substance abuse and mental health field preferredExperience in conflict resolutionCompassionate and caring demeanorAbility to build rapport with clientsExcellent written and verbal communication skillsFamiliar with community resourcesExperience with KIPU EMR preferredAbilities/Competencies:Knowledge of principles and practices of the discipline.Adheres to: Healthcare Code of EthicsKnowledge of Joint Commission StandardsKnowledge of State & Federal Statutes Regarding:Client ConfidentialityDrug-Free WorkplaceCorporate Integrity & ComplianceDemonstrate knowledge of Substance Abuse and/or Mental Health experience
