Patient-Centered Care & Clinical Excellence Conduct comprehensive
initial assessments to determine individualized home care needs.
Develop, implement, and continuously evolve patient-centered care
plans. Perform ongoing nursing assessments and adjust care plans in
real time based on changing conditions. Administer treatments,
medications, and interventions as prescribed—bringing
hospital-quality care to the home. Empower patients and families
through health education and hands-on guidance. Support proactive
discharge planning to ensure seamless care transitions. Serve as
Case Manager when designated, coordinating holistic care and
ensuring continuity. Communication & Collaboration Maintain clear,
timely, and accurate clinical documentation. Collaborate closely
with physicians, care coordinators, and interdisciplinary teams.
Serve as a vital liaison between the patient, family, and
healthcare providers. Participate in case conferences and advocate
for optimal patient outcomes Additional Duties Participate in
on-call rotation for 24/7 care continuity. Coordinate necessary
equipment, services, and resources for effective home treatment.
Supervise and evaluate home health aides every 14 days, ensuring
top-tier care delivery. Qualifications & Must-Haves Graduate of an
accredited nursing program (Associate or BSN preferred). Active RN
license in the state, current CPR certification, and valid driver's
license. 1–2 years of acute care experience; home health care
experience preferred. Strong communication, critical thinking, and
clinical documentation skills. Capable of lifting, positioning, and
transferring patients—hands-on and ready for action. Perks:
