We are seeking a Board-Certified Primary Care Provider (IM/FM) with at least 2-3 years of Medicare Advantage experience.
Overview:

• 
  
• Monday-Friday 8-5 no weekends
  
• Busy from Day 1, Existing Patient Panel
  
• Develop patient strategies to reduce hospitalization, ER visits
  
• Minimal Call, Phone call only
