We look forward to hearing from you soon! *** Bilingual (English- Spanish) a plus for this role ***OVERVIEW: The Clinical Patient Care Coordinator provides the administrative support to the clinical care management services to ensure the patient is an active participant in their care, and they receive all services appropriate for their unique needs with a focus on healing injury and restoring patients back to their desired lifestyle. The Clinical Patient Care Coordinator works as an essential team member in the clinics to provide clinical case management essential to the assessment, promotion, maintenance and restoration of patient-'s health and wellness using established protocols, clinical advice algorithms, and clinical case management within scope of practice. This role is intended to enhance the quality of patient management, maximize satisfaction, and promote patient satisfaction. ESSENTIAL DUTIES AND RESPONSIBILITIES:
      • Answer all calls in a welcoming manner with a professional, upbeat, friendly attitude.
      • Follow prepared scripts based on call type.
      • Register new patients by gathering pertinent information and entering it into EHR system.
      • Verify coverage and benefit information.
      • Schedule patient appointments upon request and/or per established care plan.
      • Document accurately and efficiently within the EHR.
      • Properly obtain and confirm patients/caller information abiding to HIPAA law.
      • Gather all necessary and pertinent patient concerns/information to facilitate the timely and accurate clinician treatment/follow up.
      • Complete prescription refills as authorized by prescribing provider.
      • Complete any necessary referrals requested for provider approval.
      • Triage patients to assess condition and provide clinical guidance using sound clinical judgement, clinical tools, and other approved resources under the supervision of iRISE medical providers in accordance with established company policies and protocols.
      • Document all conversations regarding patient care, verbally or in writing, in the EHR system.
      • Schedule appointments using appropriate clinical judgement for prioritization within company scheduling protocols.
      • Coordinate and communicate individualized and comprehensive care management and treatment plans.
      • Clearly explain prescribed treatments and treatment plans to patients.
      • Educate patients on health conditions, treatment plans, benefits, services, and other resources, including payment options when necessary.
      • Works closely with surgery coordinators and contact center representatives to educate and assist with matters that involve clinical decision making.
      • Communicate with patients, caregivers, physicians, attorneys, insurance companies, outside providers of care, treating offices, and internal departments to ensure successful outcomes.
      • Follow case and quality management processes in compliance with regulatory guidelines and company procedures.
      • Work closely with interdisciplinary care team and technology to identify high-risk, complex patient cases; manage care, including referral management, care planning, pre-op/post-discharge/post-op planning and coordinating community-based and transitional care.
      • Conduct individualized clinical evaluations of patients, their health needs and concerns; develop personalized action plans, provide education and issue referrals for care management programs; monitor patients-' progress in meeting established goals and modify care plans accordingly.
      • Uses evidence-based, comprehensive care plans to manage patients.
      • Develop a cohesive and strong team-oriented relationship with physicians, nurses, medical assistants, scribes, technologist, technicians, and other healthcare professionals; evaluate and report on patient health outcomes and work in partnership with interdisciplinary healthcare team to facilitate the best in patient care.
      • Works closely with on-site clinic teams, providing assistance with patient communication, answering phones, handling checking in and out-patients when necessary.
      • Manage for after-hours calls according to company policy, following established guidelines, clinical protocols, and escalation procedures.
      • Provide outstanding customer service in support of our mission, goals, and values.
      • Meet established standards within all performance criteria.
      • Attendance of training/meetings as needed.
      • All other duties as assigned. EDUCATION, EXPERIENCE, and TRAINING:
        • Minimum requirement of 1-2 years of healthcare and/or Medical Assistant/Nursing experience; experience in medical triage, orthopedics, and/or interventional pain management preferred.
        • Working knowledge of applicable clinical practice, preferably in a personal injury or comparable industry, is required. CERTIFICATES, LICENSES, REGISTRATIONS:
          • Relevant and comparable medical experience will be considered SUPERVISORY RESPONSIBILITIES:
            • This job has no supervisory responsibilities.KNOWLEDGE:
              • Knowledge of human anatomy & physiology, common health conditions and medical treatments. SKILLS:
